beINvolved - Form: Travel Form 2026-2027

Travel Forms must be submitted at least 1 week prior to departure.

You will receive two email notifications from this form:

First, a confirmation of submission will be automatically sent to your email. If you do not receive this, you have not submitted the form. Check your submission history - anything listed as "in progress" has not been submitted.
Second, the Club Sports staff will send notice of approval, or notice of missing waivers/forms.

Notes:

Ensure everyone traveling for your club has submitted a waiver. Email clubsprt@iu.edu if you would like to confirm who has a waiver on file for your club.
Drivers of any vehicle (personal or rental) should complete an MVR check once per academic year.
If using a personal vehicle, a Personal Vehicle Use Form must also be submitted each academic year.

Do you need letters for Professors?
The information provided on this form will be used to create a PDF letter with the names of all travelers. That PDF will be emailed to you to distribute to your members.

I understand it is Rec Sports policy that all club members must sign a waiver prior to any engaging in practices. Therefore, everyone who is traveling should already have a waiver on file. Repetitively having missing waivers will be considered violation of this policy.

I understand that this form and any requested professor letters will not be approved until all waivers are submitted. Failure to submit necessary waivers prior to departure will result in sanctions and/or suspension.
