Travel Form 2024-2025

Travel Forms must be submitted at least 1 week prior to departure.  

You will receive two email notifications from this form:

  1. First, a confirmation of submission will be automatically sent to your email.  If you do not receive this, you have not submitted the form.  Check your submission history - anything listed as "in progress" has not been submitted.
  2. Second, the Club Sports staff will send notice of approval, or notice of missing waivers/forms.
Notes:
  • Ensure everyone traveling for your club has submitted a waiver.  Email clubsprt@indiana.edu if you would like to confirm who has a waiver on file for your club.  
  • Drivers of any vehicle (personal or rental) should complete a MVR check once per academic year.
  • If using a personal vehicle, a vehicle authorization form must also be submitted each academic year.
